Forum Health At Home HOSPICE (formerly Hospice of Northeast Ohio) is a locally owned, not-for-profit organization that serves Mahoning, Trumbull and Columbiana Counties.  We have more than two decades of experience in proving an extraordinary level of care for people in the final phase of a terminal illness and their families. We provide services to everyone in need of hospice services, regardless of race, creed, gender, disability, national origin, religion or ability to pay.

Forum Health At Home HOSPICE understands the special medical emotional and spiritual needs of patients with incurable illnesses and their families. We celebrate the quality of life and emphasize living each day to the fullest by providing compassionate care and comfort for the mind, body and spirit.

Hospice Care

Hospice is a philosophy of care that can begin when curative treatment for life-limiting illness is no longer effective. The heart of hospice focuses on enhancing the quality of life with care usually provided in the familiar surroundings of home or other home-like setting. Forum Health At Home Hospice endeavors to foster choice about care decisions, to provide comfort and to preserve dignity.

Admission Criteria.

Hospice care can begin when curative treatment is no longer effective. Our criteria for admission includes:

  • Patient has a life-limiting illness
  • Life expectancy is measured in months rather than years
  • Patient and family are ready for comfort care and desire hospice
  • Attending physician agrees to manage hospice care

Our Patients

Forum Health At Home HOSPICE opens their doors and their hearts to individuals of any age with life-limiting illness.  We care for people with, but are not limited to, the following illnesses:

  • Cancer
  • Dementia/Alzheimer’s disease
  • Heart disease
  • Kidney disease
  • Neurological diseases
  • End stage lung disease
  • AIDS
  • Liver disease
  • Stroke/Coma

Our Hospice Volunteers

Hospice volunteers are a valuable part of the our caring Hospice team. They are remarkable people who give of themselves and their time in countless ways. Our volunteers lend a hand, run errands, sit with loved ones, give a caregiver respite and provide support and understanding.

Our Hospice Team and Services

Comfort, assurance and personalized care are provided by an experienced interdisciplinary team of hospice professionals, which includes physicians, nurses, social workers, therapists, personal aids, spiritual counselors and volunteers. Our hospice medical director is board certified in both hospice and palliative care.

Besides attending to the patient’s medical needs, our compassionate staff lends support, offers respite care, helps the patient and family cope with despair and grief, and helps the patient live the final days with dignity and peace.

Our hospice services include:

  • Nurses (On-call 24 hours a day)
  • Skilled nursing care
  • Short-term inpatient support for symptom management
  • Medical supervision
  • Medications to manage pain and symptoms
  • Personal care
  • Physical, occupational and speech therapy
  • Social services
  • Nutritional counseling
  • Medical supplies and equipment
  • Music relaxation / Pet therapy
  • Volunteer support
  • Respite care (to give caregivers relief)
  • Bereavement care (adults and children)
  • Spiritual support

Reimbursement

Medicare, Medicaid and most private insurance companies include a hospice care benefit. A hospice team member can help you understand your insurance coverage. Patients are accepted by Forum Health Hospice based on their need, not their ability to pay.
