General Information & Scheduling
Austintown Imaging Center is part of Forum Health Medical Imaging Services. The following information may be helpful for scheduling imaging procedures at Austintown Medical Park.

Pertinent information required for scheduling includes:
· Patient name
· Phone number
· Birthdate
· Ordering Physician
· Diagnosis
The following tests must be scheduled:
· CT
· Ultrasound
· Mammography
· Open MRI
· IVP
· BE
· GI
· Small Bowel
· Esophogram
Routine X-rays need not be scheduled and can be done on a walk-in basis at the patient’s convenience from 8:30 a.m. to 5 p.m., Monday through Friday.
All imaging procedures require a written prescription, with a diagnosis from the ordering physician. These may be faxed in or brought in by the patient.
Typed reports are normally available within two to four hours and may be electronically faxed. Final results may be used as preliminary readings upon request.
Significant positive outpatient results will be automatically reported to the physician’s office and, in such cases, patients will be detained until the physician is notified.
With 24 hours’ advanced notice, we can release films to patients or physicians.
We understand that your time is valuable, and we will make every effort to process all of our patients as quickly and efficiently as possible.
Outpatient Phone-In Registration
Patients scheduled for an outpatient visit to Austintown Imaging Center at Austintown Medical Park can pre-register by phone up to 30 days before the scheduled test or procedure by calling Forum Health Central Scheduling or the Austintown Imaging Center, per above information.
When you call, please have the following information available:
• All health cards, including policy number and group number
• The type of test or procedure needed.
• An established diagnosis; complaint or symptoms.
• The scheduled time of the test/procedure.
• The name of the physician who ordered the test/procedure.
When you come in, stop at the Registration Desk and wait for the next available registrar. You will be asked for your insurance cards, the physician’s order/script and your signature for all the appropriate paperwork. You will then be directed to your testing department.
