Forum Health At Home Hospice
Forum Health At Home Hospice offers 24-hour care, 7 days a week, to terminally ill patients and their families. It is caring, not only for the physical, but also the emotional, social and spiritual needs. The staff provides care to manage pain and other symptoms. Our goal is to allow patients to remain at home. Hospice celebrates the quality of life, not just the length of life.


The Hospice Philosophy

Hospice of Northeast Ohio was established in 1982. It has been our tradition to provide complete home health services to individuals and families in our community. Hospice is a special kind of care designed to provide sensitivity and support for people in the final phase of a terminal illness. Hospice care seeks to enable patients to carry on an alert, pain-free life and to manage other symptoms, so that their last days may be spent with dignity and quality at home or in a home-like setting, such as an assisted living facility or nursing home. Forum Health At Home Hospice is an active member in both the National Hospice and Palliative Care Organization and the Ohio Hospice Organization.


The Hospice of Northeast Ohio Team

The Forum Health At Home Hospice team includes physicians, advanced practice nurses certified in Palliative Care, hospice-certified registered nurses, hospice-trained home care aides, therapists, social workers, volunteers, clergy and pastoral counselors. Hospice services are available to persons who can no longer benefit from curative treatment. The typical hospice patient has a life expectancy of six months or less. Hospice professionals provide medical and support services, not only to the patient, but to the patient's family and loved ones. Forum Health At Home Hospice is accredited by the Joint Commission on Accreditation of Healthcare Organizations. Referrals to Hospice of Northeast Ohio usually come from the primary care physician, specialists, family, friends, clergy or other health professionals.

Hospice Care Insurance Coverage

Hospice care is a covered benefit under most private insurance plans, including HMOs and managed care companies. In addition, hospice is a covered Medicare benefit and in Ohio is a covered Medicaid benefit.


How to Become a Hospice Volunteer

If you are interested in becoming a volunteer for Forum Health At Home Hospice, we'd love to hear from you. Without our volunteers, we could never provide the high quality, end-of-life care that brings hope and consolation to patients and families. Volunteers may be asked to sit with a patient for a few hours, run errands, pick up prescriptions or do a little shopping. If you are interested in reaching out to families in need, please give us a call at (330) 841-5440.

All training materials are furnished. There is no charge for classes. We do accept volunteers already trained by other Hospice programs. During training you will learn:
